This website has one form, for asking the clinic for a visit. Here is what happens to what you type.
What we collect
Only what you enter: the reason for your visit, your name, your mobile number, an email address if you add one, the day you prefer, and your smile check result if you took it.
Where it goes
The form is sent through FormSubmit (formsubmit.co), a form-to-email service, to the clinic's inbox. It is not sold or shared with anyone else, and it is not used for advertising.
The smile check
The five questions run in your browser. Your answers are not sent anywhere unless you send the booking form, which then includes the result so the clinic knows what you'd like to talk about.
Calls and WhatsApp
Call and WhatsApp buttons open your phone's own apps. Those conversations are between you and the clinic.
Maps and fonts
The map is a Google Maps embed and the fonts come from Google Fonts, so Google may see that you opened this page. The site sets no cookies of its own and uses no tracking.
